Job summary

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will perform after-hours assessments, admissions, routine visits, and live discharges, maintaining primary clinical oversight for assigned patients.

You will lead the Interdisciplinary Group, educate families on hospice care, and ensure timely documentation and regulatory compliance while collaborating with physicians and field staff.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person patient care during after-hours, weekends, and on-call shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Support the IDG process via chart reviews and care plan updates.
• Develop and revise Hospice Plan of Care with physicians and IDG.
• Educate patients and families on disease progression and symptom management.
• Maintain accurate clinical documentation and notes.
• Coordinate scheduling and visit allocations as needed.
• Ensure infection control and compliance standards.
