Job summary

The position is 40 hours weekly with a Sunday–Wednesday night schedule, non-exempt status, and overtime eligibility. A base hourly rate is offered with differential pay potential.

Qualifications

• Must be at least 18 years of age.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional stay beyond scheduled shift until coverage is in place.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene needs per individual plans.
• Provide continence care as needed and encourage independence and skill development.
• Monitor and document health information and medications per training and directives.
• Transport individuals to appointments and community activities using agency vehicles.
• Prepare meals and snacks according to menus and dietary requirements.
• Document daily activities and medication administration in the electronic health record (EHR).
• Follow emergency procedures and safety protocols; support behavioral needs with trained techniques.
• Communicate effectively with colleagues, supervisors, individuals served, and families.
