Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver’s license with transportation ability.
• Able to pass background checks, fingerprinting, health screenings.
• Complete required training and certifications.
• Commitment to safety and professional conduct.
• Occasional extended shifts to ensure coverage.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene per plans.
• Monitor health info and document vitals as needed; administer medications per directives.
• Transport individuals to appointments, events, and activities in agency vehicles.
• Prepare meals and assist during meals per menus and dietary needs.
• Assist mobility and transfers using approved techniques and equipment.
• Document daily activities, health data, incidents, and medications in EHR.
• Follow emergency protocols and apply basic first aid/CPR when required.
• Collaborate with team, attend meetings, and participate in trainings.
