Oak Hill is seeking a Direct Support Professional to assist individuals with daily living, health, and community participation in Harwinton, CT. The role emphasizes compassion, safety, and teamwork on a night shift schedule (Sunday-Wednesday 11pm-9am) with a 30-hour work week.

You'll provide personal care, monitor health, administer medications as trained, transport to appointments, prepare meals, document health information in the EHR, and participate in training and meetings to support

Qualifications

• Must be at least 18 years old and able to obtain background checks.
• High school diploma or GED required.
• Valid driver’s license with transportation ability.
• Ability to complete required trainings and certifications.
• Commitment to safe, healthy, professional work environment.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene.
• Provide continence care as needed and promote independence.
• Monitor health information and document vital signs when required.
• Administer medications per training and nursing directives.
• Support mobility, transfers, and use of adaptive equipment.
• Document daily activities and notes in the electronic health record (EHR).
• Transport individuals to appointments and community activities.
• Prepare meals and assist during mealtimes according to dietary needs.
• Respond to emergencies using established safety protocols and First Aid/CPR if required.
• Work effectively in a team and participate in trainings and meetings.
