Job summary

Oak Hill in Simsbury, CT is seeking a Direct Support Professional to assist individuals with daily living, health monitoring, nutrition, and community participation. You’ll document health information in the EHR, support mobility, and follow care plans with compassion.

The role offers a 30-hour week, paid training, certifications within 90 days, and benefits. This is a union, non-exempt position with overtime eligibility and opportunities for career mobility.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to safety and Oak Hill policies.
• Commitment to continuity of care, including occasional overtime or staying beyond scheduled shifts.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene per support plans.
• Monitor health information and document vitals when required.
• Transport individuals to appointments and community activities using agency vehicles.
• Prepare meals and snacks per menus and dietary needs.
• Assist mobility and transfers using approved methods and equipment.
• Document daily activities, health information, progress, and medication administration in the EHR.
• Follow emergency procedures and apply basic first aid/CPR when needed.
• Communicate effectively with team, individuals served, families, and stakeholders.
