Job summary

UnityPoint Health in Des Moines, IA, seeks an LPN for the Med/Surg Y8 unit for full-time or part-time night shifts from 7pm to 7:30am, with every third weekend and holiday rotation.

The Younker 8 unit cares for a variety of acute care patients, collaborating with Residency programs to advance patient care. The team emphasizes collaboration, teamwork, and innovation in delivering care.

Qualifications

• Successful completion of a licensed practical nurse program from an accredited college.
• Current licensure as an LPN in Iowa in good standing.
• BLS certification or ability to obtain within three months of hire.
• Preferred experience in Post-Acute or Acute Care settings.

Responsibilities

• Provide nursing care for patients with disability and chronic illness.
• Educate patients and families and develop discharge plans.
• Perform hands-on nursing care using the nursing process to achieve outcomes.
• Supervise unlicensed assistive personnel and guide care delivery per LPN scope.
• Coordinate care with the rehab team to meet goals.
• Assess physical, psychological, sociocultural, and spiritual needs of patients and families.
• Implement and adjust the interdisciplinary plan of care within LPN scope.
• Evaluate nursing care and modify plans to meet objectives.
