UnityPoint Health—Younker 7 unit is seeking a Licensed Practical Nurse for our Med/Surg team. This role supports patients with acute and chronic conditions, performing hands-on nursing care and patient education in a collaborative setting.

Qualifications

Must have completed an LPN program.
Current Iowa LPN license in good standing.
BLS certification or obtain within 3 months of hire.

Responsibilities

Provide nursing care per the nursing process for patients with acute and chronic conditions.
Educate patients and families and develop individual discharge plans.
Coordinate care with the interdisciplinary team to meet goals.
Supervise and guide unlicensed assistive personnel as needed.
Contribute to ongoing assessment of patients’ physical, psychological, and social needs.
Implement the plan of care within LPN scope and adjust as needed to prevent complications.
Participate in performance improvement activities to achieve measurable outcomes.
