Job summary

Pioneer Human Services is hiring a Health Coordinator to support health services in residential treatment facilities during the night shift. The role focuses on coordinating physical and behavioral health care, facilitating appointments (including telehealth), and ensuring accurate medication documentation in eMAR/MAR.

You will work under nursing staff where available and contribute to continuity of care. Candidates will assist with intake, provide direct client support, monitor health changes,

Qualifications

• High School Diploma or equivalent is required.
• 1 year of experience in caregiving, human services, behavioral health, or with vulnerable populations.
• Must apply for Agency Affiliated Counselor registration within 30 days of hire and obtain/maintain within 90 days.
• First Aid/CPR certification within 90 days of hire.
• Strong written and verbal communication skills; attention to detail.
• Basic computer skills and ability to learn new systems.

Responsibilities

• Coordinate and monitor client physical and behavioral health services within residential facilities.
• Assist with intake, telehealth appointments, and medication monitoring with MAR/eMAR records.
• Support care coordination under nursing supervision where available; otherwise under Health & Resource Coordination Manager.
• Provide direct client support within a structured residential environment and reinforce treatment plans.
• Maintain accurate documentation in the electronic health record and ensure timely updates.
• Ensure safety and quality of care, escalate issues to clinical staff as needed.
