Job summary

Pioneer Human Services is seeking a Health Coordinator (FT/Swing) to support residential treatment facilities by coordinating physical and behavioral health services, facilitating communication among clients, nursing staff, physicians, and program staff.

The role includes intake support, telehealth appointment coordination, medication monitoring, and health information relay to clinicians, with supervision depending on site nursing presence.

Qualifications

• High School Diploma or Equivalent required.
• 1 year of experience in caregiving, human services, behavioral health, or social services setting.
• First Aid/CPR certification within 90 days of hire.
• Proficient written and verbal communication; ability to manage tasks independently.

Responsibilities

• Coordinate client physical and behavioral healthcare services during shifts, including telehealth and clinic visits.
• Coordinate intake and discharge processes after hours, including financial documentation.
• Maintain accurate documentation in the electronic health record and ensure timely updates.
• Assist with medication handling, storage, and coordination with nursing staff where applicable.
