Job summary

Pioneer Human Services is seeking a part-time Health Coordinator to support clinical health services within residential treatment facilities. You will coordinate medical and behavioral health appointments, assist with intake processes, and help ensure continuity of care for clients in recovery.

Responsibilities include coordinating care plans, documenting health information, and providing safe, patient-centered support under nursing or manager supervision as needed.

Qualifications

• High School Diploma or Equivalent.
• One year of experience in caregiving, human services, behavioral health, or social services setting.
• First Aid/CPR certification within 90 days of hire.
• Must apply for Agency Affiliated Counselor registration within 30 days of hire and obtain within 90 days of hire.
• Proficient written/verbal communication and attention to detail.

Responsibilities

• Support client physical and behavioral health services within residential facilities.
• Coordinate intake and discharge processes and telehealth visits.
• Assist with medication monitoring and documentation accuracy in eMAR/MAR.
• Provide direct client support in a structured residential environment.
• Maintain accurate documentation in the electronic health record.
