Job summary

Pioneer Human Services is seeking a Health Coordinator to support residential treatment facilities by coordinating physical and behavioral health services and ensuring continuity of care for clients.

The role includes intake coordination, appointment scheduling (including telehealth), medication monitoring, and documentation in the electronic health record. Strong organization, communication, and the ability to work in a structured environment are essential.

Qualifications

• High School Diploma or equivalent required.
• One year of experience in caregiving, human services, behavioral health, or social services setting.
• Must apply for Agency Affiliated Counselor registration within 30 days and obtain/maintain active registration within 90 days.
• First Aid/CPR certification within 90 days of hire.
• Demonstrated written and verbal communication skills and attention to detail.

Responsibilities

• Coordinate client physical and behavioral health services across residential facilities.
• Assist with intake, appointments, telehealth, and medication monitoring.
• Document health information in the electronic health record and ensure continuity of care.
• Support care coordination under nursing staff direction or Health & Resource Coordination manager.
• Provide direct client support in a structured environment and respond to urgent health concerns.
