Qualifications

• RN licensed in NJ with at least 5 years of clinical experience.
• Strong organizational, time-management and critical-thinking abilities.
• Proficiency with MS Office and electronic medical records.
• BSN preferred; NJ license required.

Responsibilities

• Coordinate outpatient care and identify new patients requiring navigation.
• Assess patient and family needs and educate; connect to resources.
• Develop survivorship care plan for applicable patients and discuss with team.
• Build relationships with surgeons and providers; coordinate with office staff.
• Prepare and document multidisciplinary conferences; promote treatment planning.
• Participate in committees; ensure standards and Joint Commission compliance.
• Represent program to community; assist with brochures and outreach.
