Job summary

Hackensack Meridian Health is seeking an Oncology Nurse Navigator to guide oncology patients from pre-diagnosis through survivorship. Working with the multidisciplinary team, you will coordinate outpatient care, referrals, and access to services to ensure timely and comprehensive treatment.

The role requires a BSN, at least two years of nursing experience, and NJ RN license. Oncology Certified Nurse preferred.

Qualifications

• Excellent written and verbal communication skills.
• Proficient computer skills that may include Microsoft Office and/or Google Suite platforms.

Responsibilities

• Coordinate and navigate outpatient care for oncology patients from pre-diagnosis through survivorship or end of life.
• Establish communication with physicians, physician extenders and their staff to outline navigation programs and resources.
• Build and maintain relationships with medical oncologists, radiation oncologists, surgeons and other professionals involved in patient care.
• Facilitate access to services and track patient flow through diagnostic and treatment services; document in medical records.
• Participate in performance improvement activities, develop guidelines with Medical Directors and leadership to improve patient outcomes.
• Represent the Cancer Program's services to the community and collaborate with marketing for outreach efforts.
