Job summary

Hackensack Meridian Health is seeking a Nurse Navigator to coordinate and oversee outpatient care, collaborating with surgeons, physicians, and the care team in New Jersey. You will assess patient and family needs, connect them with community resources, and contribute to survivorship planning.

This role requires RN licensure and at least five years of clinical experience, with strong MS Office skills and independent judgment. A BSN is preferred, and a comprehensive benefits package is offered.

Qualifications

• Graduate of licensed school of nursing.
• RN with at least 5 years of clinical experience in a relevant clinical specialty.
• Proficient computer skills—proficiency in MS Office.

Responsibilities

• Coordinate and navigate outpatient care for patients.
• Identifies new patients as they are diagnosed and offers navigation services.
• Provides initial detailed patient and family needs assessment and periodically updates needs assessment.
• Develops disease site expertise.
• Provides patient and family education.
• Connects patients and family with available resources in the health system and community.
• Maintains a database of available community resources.
• Facilitates access to services.
• Documents assessment and follow up notes in the patients' medical record.
• Provides monthly reports of activity to the Director.
