Job summary

Hackensack Meridian Health Inc. seeks a nurse navigator to coordinate patient care with surgeons and the care team. The role includes assessing patient and family needs across educational, financial, psychosocial, nutritional, and clinical areas, and connecting families to system and community resources.

Required is an RN with at least 5 years of clinical experience, NJ RN license, BLS, proficiency in MS Office, and strong organizational and judgment skills. BSN is preferred for this role.

Qualifications

• Graduate of licensed school of nursing.
• RN with at least 5 years of clinical experience in a relevant clinical specialty.
• Proficient in MS Office.
• Strong organizational, time management, and critical thinking RN skills.
• Ability to work independently and exercise sound judgment in interactions with physicians, APNs, patients, and their families.
• BSN preferred.
• NJ State RN license and BLS certification.

Responsibilities

• Nurse navigator collaborates with surgeons and providers to coordinate patient care.
• Ongoing assessment of patient and family needs across educational, financial, psychosocial, nutritional, and clinical areas.

The nurse navigator in collaboration with the surgeons and/or providers and the entire healthcare team navigates the care for patients including but not limited to: ongoing assessment of patient and family needs including educational, financial, psychosocial, nutritional, and clinical; connecting patients and families to available resources within the health system and in the community, assessing patients for eligibility for clinical trials and facilitates access to care.

Education, Knowledge, Skills and Abilities Required:

• Graduate of licensed school of nursing;
• RN with at least 5 years of clinical experience in a relevant clinical specialty.
• Proficient computer skills- proficiency in MS Office.
• Strong organizational, time management, critical thinking RN skills.
• Ability to work independently and exercise sound judgment in interactions with physicians, APNs, patients, and their families.

• BSN

Licenses and Certifications Required:

• NJ State Professional Registered Nurse License.
• BLS AHA Basic Health Care Life Support HCP Certification.
