Job summary

Hackensack Meridian Health is seeking a Palliative Nurse Navigator to collaborate with palliative care providers to deliver high-quality care to patients and families. This role includes assessment, education, and care coordination across the health system and community.

The ideal candidate is an RN with 5+ years of relevant clinical experience, proficient in EPIC and Google/Microsoft tools, and capable of working independently with strong communication skills. A BSN is preferred.

Qualifications

• RN with at least 5 years of clinical experience in a relevant specialty.
• NJ RN license required; BSN preferred.

Responsibilities

• Coordinate and navigate palliative care for patients and families.
• Assist with advance care planning and symptom management.
• Educate and collaborate with providers and staff.
• Participate in committees and quality initiatives.
• Support transitions of care and palliative services.
• Provide education to community organizations about palliative care.
• Adhere to organizational competencies and standards.
