Job summary

Hackensack Meridian Health’s Neurology Outpatient team seeks a Patient Navigator to optimize the patient journey, remove barriers, and empower patients with resources to confidently manage their care, appointments, and follow-ups.

Under the direction of practice leadership, you will support the clinical team, streamline workflows, and enhance the patient experience while ensuring clear communication between patients and providers.

Qualifications

• Minimum of a Bachelor's Degree.
• Minimum 2 years as a Registered Nurse.
• Proficient Computer Skills that may include, but are not limited to, Google Suite Platforms.
• Excellent written and verbal communication skills.
• Strong organizational and critical thinking skills.
• Must be organized and have the ability to multitask, working in a fast-paced environment.
• Must have the ability to work independently, but also in a team environment.

Responsibilities

• Role focuses on optimizing the patient journey and driving positive outcomes.
• Act as a dedicated advocate, streamlining care processes and facilitating communication between patients and providers.
• Provide key support to the clinical team, ensuring a smooth, coordinated workflow and greater patient satisfaction.
