Job summary

The candidate will have an RN license in New Jersey with at least 5 years of clinical experience, strong communication skills, and proficiency in EPIC and Microsoft Office.

Qualifications

• RN degree or equivalent with NJ RN license.
• BSN preferred.
• Experience in palliative or related specialty is desirable.

Responsibilities

• Coordinate and navigate palliative care for patients.
• Facilitate access to palliative care services within the health system.
• Educate patients and families about palliative care options.
