Job summary

Hackensack Meridian Health seeks a Nurse Navigator for the Structural and Congenital Heart Program. This role acts as an advocate, educator, and facilitator of care for patients and families across the continuum—from initial consultation through preoperative preparation, hospital admission, and postoperative follow-up.

Qualifications

• Graduate of a NLN/AACN accredited program in nursing.
• RN with at least 5 years of clinical experience in a relevant clinical specialty.
• Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
• Strong organizational, time management, critical thinking RN skills.
• Ability to work independently and exercise sound judgment in interactions with physicians, APNs, patients, and their families
• Excellent written and verbal communication skills.

The Nurse Navigator, Structural & Congenital Heart Program, is an advocate, educator, and facilitator of care for the Structural and Congenital Heart patient and their family, throughout the continuum of care, form point of initial consultation, through preoperative preparation, admission, hospital stay, and postoperative evaluation.

Education, Knowledge, Skills and Abilities Required:

• Graduate of a NLN/AACN accredited program in nursing.
• RN with at least 5 years of clinical experience in a relevant clinical specialty.
• Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
