Job summary

Hackensack Meridian Health is seeking a Nurse Navigator in Neptune, NJ to collaborate with surgeons and the healthcare team, addressing educational, financial, psychosocial, nutritional, and clinical needs. This role supports patients through their treatment journey.

The ideal candidate has 5+ years of clinical experience in surgical nursing and related fields, with NJ RN licensure and BLS/HCP certification. Day shift, full-time position located in Neptune, NJ.

Qualifications

• RN with clinical experience in surgical nursing or related field.
• Strong patient/family education and care coordination skills.
• Licensed in NJ with BLS/HCP certification.

Responsibilities

• Navigate outpatient care for patients and identify newly diagnosed patients.
• Perform initial detailed patient and family needs assessments and update them periodically.
• Develop disease site expertise and provide patient and family education.
• Connect patients and families with resources within the health system and community.
• Maintain a database of available community resources and facilitate access to services.
• Track the flow of patients through diagnostic and treatment services.
• Document assessments and follow-up notes in patients' medical records.
• Gather information required by the Commission on Cancer for survivorship care plans.
• Build and maintain professional relationships with surgeons and other medical professionals.
• Collaborate with physicians for multidisciplinary care conferences and ensure documentation.

What You'll Do

Requirements

Education

• Graduate of licensed school of nursing RN

Experience

• At least 5 years of clinical experience in a relevant clinical specialty (surgical nursing and quality background)

Licenses & Certifications

• NJ State Professional Registered Nurse License


• AHA Basic Health Care Life Support HCP Certification


• National Certification preferred
