Job summary

Via Care Community Health Center seeks a Neurodevelopmental Disorder Care Coordinator to support pediatric NDD patients and families as they navigate Via Care’s care system. The role links dental, primary care, and social services, advocating for patients and ensuring access to appropriate information.

The NDD-CC works in outreach and program coordination, handles Medi-Cal eligibility, and collaborates with the team to support the pediatric NDD dental program.

Qualifications

• Coordinate care for pediatric patients with neurodevelopmental disorders within a multidisciplinary team.
• Assist with linkage to case management, insurance navigation, and Medi-Cal eligibility.
• Educate families about the health care system and available services.
• Receive referrals and support program navigation for NDD dental care.
• Participate in monthly trainings and team meetings.

Responsibilities

• Operate as a health systems navigator for pediatric NDD patients and caregivers.
• Advocate for patients across dental, primary care, and supportive services.
• Maintain records, document outcomes, and arrange patient appointments.
• Collaborate with pediatric and dental providers and external social service orgs.
• Support program enrollment and monitor eligibility and referrals.
