Job description

The Neurodevelopmental Disorder Care Coordinator (NDD-CC) engages Via Care's (primarily pediatric) dental patients with neurodevelopmental disorders (NNDs) and assists with linkage to case management support and clinical services. The NDD-CC is a critical component in this program, a health care professional whose duties and responsibilities are to support pediatric NDD dental patients, caregivers, and their families as they move through Via Care's care system. The NDD Care Coordinator functions as a liaison between patients/caregivers, pediatric providers, dental providers, and external social service organizations. You help pediatric NDD patients connect with dental, primary care, and supportive services, both within and outside of Via Care — advocating on their behalf and ensuring that all parties have the appropriate patient information.

The NDD-CC is responsible for in-reach and program coordination via both in-person and telephonic modalities. The NDD-CC will also receive outside referrals, supporting all eligible patients in accessing the pediatric NDD dental program. The NDD-CC is charged with the navigation of individual members’ program eligibility involving insurance and, specifically, Medi-Cal eligibility. The NDD-CC will also be assigned to support other team members in coordination with the pediatric NDD dental program.

  • You will be required to participate in monthly meetings and NDD training courses.
  • Stay up to date with programmatic changes within Via Care’s pediatric and dental care for children with neurodevelopmental disorders.
