Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a Part-Time LCSW to provide real-world psychosocial support in home and community settings for individuals with neurological conditions. You will work autonomously within a neuro rehab team to help patients and families adapt, rebuild function, and regain independence.

Your duties include comprehensive evaluations, care coordination, resource navigation, and ongoing documentation, with flexible scheduling and mileage/time reimbursement as part

Qualifications

• Master's degree in social work or related field required.
• Current licensure in state(s) of practice required.
• Experience in rehabilitation settings preferred.

Responsibilities

• Conducts comprehensive evaluations of social, emotional, and support needs with measurable objectives alongside the rehab team.
• Identifies community resources to achieve objectives and address psychosocial needs.
• Provides direct social work interventions and counsels patients and families in coping with injury/illness.
• Regularly re-assesses progress and modifies goals or treatment as needed.
• Documents evaluations, treatment, and billing in compliance with regulations.
