Rehab Without Walls Neuro Rehabilitation is seeking a Licensed Clinical Social Worker (LCSW) to support individuals and families navigating neurological conditions in home and community settings. You will deliver psychosocial support, empower clients, and collaborate with a neuro rehabilitation team to optimize daily functioning and independence.

The role offers flexible scheduling, hourly compensation, mileage drive-time pay, and opportunities for education and growth, with a $500 sign-on

Qualifications

• Master's Degree in Clinical Social Work (LCSW) required.
• Current Licensed Clinical Social Worker Licensure in the practicing state.
• Minimum of one year experience in rehabilitation settings (brain/spinal injury or acute diagnosis).
• CPR certification required.

Responsibilities

• Performs comprehensive evaluations of social, emotional, and support needs of patients and families.
• Sets measurable objectives with rehab team.
• Assist physicians and health team in understanding social/emotional factors affecting health.
• Counsels patients and families on coping with illness.
• Participates in care coordination and connects patients with resources.
• Identifies community resources to meet objectives.
