Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a Licensed Clinical Social Worker to provide real-world psychosocial support in home and community settings. This role centers on helping patients and families adapt, stabilize, and regain function after neurological injuries or illnesses.

The LCSW will work within a neuro rehab team that values autonomy, flexibility, and meaningful clinical impact, with autonomous scheduling and travel between community sites.

Qualifications

• Must hold an active LCSW license in the state where practice occurs.
• Experience delivering psychosocial support to neurological conditions in home/community settings.
• Ability to coordinate care and navigate community resources for patients and families.

Responsibilities

• Perform comprehensive social, emotional, and support assessments for patients and families.
• Set measurable objectives with the rehab team to guide recovery.
• Support physicians and health team by addressing social and emotional factors affecting health.
• Counsel patients and families on coping with illness and treatment.
• Participate in care coordination and connect patients with community resources.
