Rehab Without Walls Neuro Rehabilitation in Hollywood, FL is seeking a PRN Licensed Clinical Social Worker (LCSW) to join our Home & Community Program. You will provide psychosocial assessments, emotional support, and help patients navigate recovery in home and community settings.

Collaborate with a multidisciplinary team, connect clients with resources, advocate for their needs, and support care coordination.

Qualifications

• Master's degree in Clinical Social Work required.
• Current LCSW licensure in state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis.
• Current BLS, First Aid and CPR certifications.
• Ability to lift 50 pounds (up to 100 pounds with assistance) and related physical requirements.

Responsabilités

• Provide psychosocial assessments and emotional support for patients and families.
• Assist with coping strategies, adjustment, and community reintegration.
• Connect patients with community resources and support services.
• Collaborate with therapists, physicians, case managers, and caregivers to support treatment goals.
• Advocate for patients throughout their rehabilitation journey.
• Performs care coordination activities and identifies patient needs.
