Job summary

Rehab Without Walls Neuro Rehabilitation in Miami, FL is seeking a PRN Licensed Clinical Social Worker (LCSW) to support our Home & Community Neuro Rehabilitation program. You will provide psychosocial assessments, support coping strategies, and help patients reconnect with important activities and relationships.

You will collaborate with therapists, physicians, case managers, and caregivers to advance treatment goals, advocate for patients, and connect families with community resources,

Qualifications

• Master's Degree in Clinical Social Work (LCSW) required.
• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury or acute diagnosis.
• Current BLS First Aid and CPR certification required.
• Ability to lift 50 pounds; moving/transitioning patients may require lifting up to 100 pounds.
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.

Responsibilities

• Performs comprehensive evaluations of social, emotional, and support needs for patients and families.
• Sets measurable objectives with rehab team.
• Assists health team in understanding social/emotional factors related to health problems.
• Instructs and counsels patients and families on coping with illness.
• Participates in care coordination and acts as a resource to health team.
• Identifies and connects patients/families to community resources to meet objectives.
