Job summary

BrightSpring Health Services in the Greater Sacramento Region is seeking an experienced Physical Therapist to provide functional rehabilitation wherever life happens—home, school, workplace, or the community.

You’ll work autonomously within a collaborative team of therapists, with a flexible, self‑created schedule, no weekends or holidays, and paid hourly compensation plus mileage reimbursement.

This role offers growth and advancement opportunities across our home health and community program.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted PT license in the practicing state.
• Current BLS/CPR certification.
• One year of PT experience preferred.
• Knowledge of neuro rehab techniques.
• Effective verbal and written communication.
• Ability to lift 50 pounds; up to 100 pounds with safety.
• Health screen may be required prior to assignment.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess progress and attend staffing/family conferences.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team, physician, and family.
• Collaborate with other disciplines to coordinate care and improve outcomes.
