Job summary

BrightSpring Health Services is seeking a Physical Therapist to provide rehab services in home and community settings. You will evaluate clients, develop and adjust treatment plans, and collaborate with a multidisciplinary team across various environments.

The role emphasizes autonomy within a supportive, team-driven culture and requires strong communication and clinical skills. Candidates should hold a PT license, a bachelor’s degree, CPR/BLS certification, and be able to lift up to 100 pounds

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support/CPR certification per state regulations.
• Minimum of one year’s PT experience preferred.
• Knowledge of rehabilitation techniques for complex neurological injury.
• Ability to communicate effectively in verbal and written form.
• Ability to lift 50 pounds; may involve up to 100 pounds.
• Physical requirements include lifting, bending, standing/walking for extended periods.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and participate in staffing/family conferences.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team, physicians, and family.
• Collaborate with other disciplines to ensure coordinated care and outcomes.
