Job summary

BrightSpring Health Services is seeking a licensed Physical Therapist to provide functional rehabilitation in home and community settings. You will evaluate patients, create and adjust treatment plans, and support clients in real-life activities such as cooking, grocery shopping, and workplace tasks.

The role emphasizes autonomous work within a collaborative therapy team, with driving/travel reimbursements, ongoing education, and opportunities for growth and advancement.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practice state.
• Current Basic Life Support (BLS) or CPR certification per state regulations.
• Minimum of one year of PT work experience preferred.
• Knowledge of rehabilitation techniques for neurological injury.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness and attend staffing/family conferences.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team and family members.
• Collaborate with other disciplines to optimize patient outcomes.
