Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a Physical Therapist to provide in-home and community neuro rehabilitation with a high degree of autonomy. You will evaluate, plan, and implement individualized neuro-focused therapy in real-world settings, promoting independence and function.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program
• Current, unrestricted Physical Therapy license in the practicing state
• CPR/BLS certification according to state regulations
• Minimum 1 year PT experience preferred
• Knowledge of neuro rehabilitation techniques
• Effective written and verbal communication
• Physical ability to lift 50 lbs and assist with up to 100 lbs
• Ability to move, lift and transfer patients per safety procedures

Responsibilities

• Perform evaluations and develop treatment plans
• Regularly reassess treatment effectiveness and attend meetings
• Conduct discharge evaluations and provide home/community safety recommendations
• Communicate patient needs and progress to the treatment team and family
• Collaborate with other disciplines to improve patient outcomes
