BrightSpring Health Services Corporation’s Rehab Without Walls Neuro Rehabilitation is seeking a Physical Therapist (PT) to provide one-on-one neuro rehab in home and community settings in the Waltham area. You will evaluate patients, develop treatment plans, and drive outcomes toward real-life independence.

Qualificações

• Bachelor's degree in Physical Therapy from an accredited program.
• Active, unrestricted Physical Therapist license in the practicing state.
• Current BLS or CPR certification as required by state regulations.
• At least 1 year of physical therapy experience preferred.

Responsabilidades

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make recommendations for home and community safety, and provide resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to ensure coordinated care and improved outcomes.
