Job summary

BrightSpring Health Services, through Rehab Without Walls Neuro Rehabilitation, is seeking a Physical Therapist to provide neuro-focused home and community PT services. You will evaluate, plan and implement individualized therapy, focusing on real-world functional outcomes.

Role emphasizes autonomy, local coverage areas, and a 1:1 patient load with travel to patient homes. Licensure and BLS/CPR are required, with prior neuro rehab experience preferred.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in practicing state.
• Current BLS/CPR certification.
• Minimum one year of PT experience preferred.
• Knowledge of rehabilitation techniques for complex neurological injuries.
• Strong verbal and written communication skills.
• Ability to lift 50 pounds and move patients up to 100 pounds.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans and attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home and community safety recommendations.
• Communicate patient's needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to coordinate care and improve outcomes.
