Job summary

Atrium Health is seeking a licensed Physical Therapist to float across Physical Therapy clinics in the Greater Charlotte area. You will provide age-appropriate care and support continuity of services across locations.

You will evaluate patients, develop evidence-based treatment plans, set functional goals with patients and caregivers, educate on home programs, and document thoroughly. Collaboration with other disciplines and adherence to policies are essential.

Qualifications

• Must hold a Physical Therapy license in the state of employment.
• BLS certification for Healthcare Providers is required.
• Continuing education requirements as mandated by state professional standards.

Responsibilities

• Provide age-appropriate patient care across patient populations.
• Perform musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations and plan care.
• Establish functional goals with patient/caregiver and monitor outcomes.
• Educate patients and caregivers and provide home programs when needed.
• Communicate professionally with patients, caregivers, and staff; attend multi-disciplinary meetings.
• Complete documentation including evaluations, progress notes, discharge summaries, and billing charges.
• Maintain a clean, organized department and ensure equipment is in good working order.
• Engage in continuing education and evidence-based practice.
• Float between departments per Enterprise Float Program guidelines.
