Job summary

The role requires age-appropriate care, excellent communication, and collaboration with providers and staff; BLS certification and ongoing CEUs are expected; travel between sites may be required.

Qualifications

• Physical Therapy license in the employing state(s).
• BLS certification for Healthcare Providers.
• Doctor of Physical Therapy (DPT) preferred.
• Continuing education requirements maintained.

Responsibilities

• Provide age-appropriate physical therapy evaluation and care.
• Develop and adjust patient care plans with evidence-based interventions.
• Collaborate with providers and staff to coordinate care.
• Educate patients and caregivers on diagnoses and home programs.
• Maintain timely documentation and billing-related records.
• Demonstrate flexibility to float across departments per program.
