Job summary

Golden Valley Health Centers seeks a PACE Care Coordinator Float RN to provide nursing services across Merced, Modesto and Stockton PACE Centers. You will assess participants, participate in IDT care planning, and coordinate care while floating to sites as needed.

The role requires strong leadership, documentation and collaboration with the care team; hours are typically Mon–Fri 8–5 with some overtime. Compensation is hourly with a daily bonus for scheduled days.

Qualifications

• RN background with clinic/home care experience preferred.
• Experience coordinating care with interdisciplinary teams (IDT).
• Strong documentation and record-keeping abilities.

Responsibilities

• Assess participants and perform nursing duties per plan.
• Float between PACE centers at Merced, Modesto and Stockton as needed.
• Coordinate with IDT to plan and evaluate care.
• Run reports for panel coordination and utilization.
• Support care planning and follow-up with participants and families.
