Job summary

Golden Valley Health Centers in California's Central Valley seeks an Outreach Specialist to plan, organize, and direct outreach and marketing efforts. The role involves intake and enrollment and coordination with the interdisciplinary team to facilitate new PACE participant enrollment.

Until the Stockton location is operational, the position will be based and trained at the Modesto center. Schedule is Monday–Friday, 8:00am–5:00pm. Compensation is $25.87–$27.16 per hour.

• Valid California Driver’s License, acceptable driving record and vehicle insurance.

• Travel to other locations as requested.

Responsibilities

• Plans and organizes outreach and marketing efforts, intake and enrollment, and coordination with the Interdisciplinary team to facilitate enrollment.
• Coordinates and executes intake and enrollment activities and processes as required.
• Identifies opportunities to collaborate with community organizations conducive to gaining eligible PACE participants.
• Establishes rapport and maintains relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities and other potential referral sources in the service area.
• Develops and implements annual marketing plan to achieve enrollment goals.
• Provides education referral sources and educates the community on PACE services.
• Participates in quality improvement.
• Participates in public relations to support goals including television, radio, press, and public speaking opportunities.
