Job summary

Golden Valley Health Centers in Merced, CA seeks a PACE Level of Care Clinic RN to conduct LOC assessments and provide clinic nursing services within the Central Valley PACE Center.

You will collaborate with the Interdisciplinary team to plan care, document DHCS-required assessments, and educate participants and families about disease management and preventive strategies. Schedule: Mon-Fri, 8am-5pm.

Qualifications

• Valid CA Driver’s License with acceptable driving record.
• Current CA Registered Nurse (RN) license and BLS CPR.
• Graduate of an accredited nursing program.
• Minimum of 1 year as an RN, preferably home/community health.
• Strong communication, organization and problem-solving skills.
• Proficient with Microsoft Office and Excel.

Responsibilities

• Conduct LOC assessments to determine participant eligibility.
• Provide clinic nursing services, care planning and care management.
• Lead and participate in the Interdisciplinary Team (IDT).
• Develop and update care plans based on nursing process and family input.
• Complete LOC documentation for DHCS review annually.
• Educate participants and families on disease processes and self-care.
• Identify emergencies and coordinate services from home when needed.
• Perform ongoing nursing assessments and document findings accurately.
