Job summary

CareSource in Mobile, AL seeks a Care Guide to join an interdisciplinary care coordination team. You will serve as a single point of contact to coordinate care, engage with members in hospital, clinic, home visits, or by phone, and identify barriers to care.

Under supervision of Care Manager, you will implement interventions, educate members about options and resources, document activities, and help plan post-discharge transitions while maintaining confidentiality and compassionate service.

Qualifications

• Associate's Degree or equivalent years of relevant work experience required.
• Minimum of one year of clinical experience in nursing, social services, or healthcare field is required.
• Licensure/certification preferred (e.g., LPN, CNA/STNA, Med Assistant, EMT, etc.).

Responsibilities

• Engage with members in a variety of settings to establish a professional relationship.
• Identify needs and prioritize efforts with the member and caregivers.
• Document care coordination activities and member responses per standards.
• Collaborate with Care Managers and providers for post-discharge planning and transitions.
• Maintain confidentiality and ethical conduct in all interactions.
