Job summary

CareSource is seeking a Community Health Worker in Mobile, AL to coordinate care as part of an interdisciplinary care team. You will engage with members in various settings, accompany them to appointments, and coordinate logistics to support care plan goals, including transportation and childcare arrangements.

Responsibilities include verifying eligibility and health status, contributing to assessments, and developing individualized care plans with case managers.

Qualifications

• High School Diploma or GED required.
• Minimum two years experience in community settings coordinating services preferred.
• Experience with at-risk populations is a plus.

Responsibilities

• Engage with members to establish professional relationships.
• Accompany members to appointments and social service encounters.
• Coordinate logistics to support care plan goals, including transportation and childcare arrangements.
• Verify eligibility, enrollment history, demographics and current health status.
• Contribute to assessments by gathering information from members and stakeholders.
• Assist with development and implementation of individualized care plans and reporting to Case Manager.
• Travel to conduct member, provider and community visits as needed.
