Job summary

Qualifications

• Nursing degree or Bachelor’s in healthcare field.
• RN, Social Worker, or Clinical Counselor licensure required.
• Minimum of 3 years in nursing, social work, counseling, or health care.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage member and natural supports using trauma-informed care and motivational interviewing.
• Coordinate ICT meetings and ensure care plan progress.
• Develop and regularly update ICP with member desires and needs.
• Identify and address barriers to care plan goals.
• Educate on treatment options and community resources; coordinate with providers.
