Job summary

CareSource in Ohio is seeking a Care Guide to join its interdisciplinary Care Coordination Team. You will coordinate care for members, serve as a single point of contact when care management involvement is requested, and help close gaps in care.

This mobile, full-time role requires travel within designated Ohio locations, strong communication, and compassionate service. You will document activities, educate members, and collaborate with providers and Care Managers to plan post-discharge care.

Qualifications

• Associate's Degree or equivalent years of relevant work experience required.
• 1+ year of clinical experience in nursing, social services, or healthcare field required.
• Medicaid and/or Medicare managed care experience preferred.

Responsibilities

• Engage with members in various settings to establish professional relationships.
• Identify needs and prioritize efforts with member and caregivers.
• Gather information to identify and manage barriers to care.
• Close gaps in care where appropriate.
• Under supervision of Care Manager, implement interventions based on standards.
• Educate member and stakeholders about treatment options and resources.
• Collaborate with Care Managers and providers for post-discharge planning.
• Document activities and member response timely per CareSource policies.
