Job summary

CommuniCare Rehab in Terre Haute, IN is seeking a Physical Therapist (PT) for a full-time split position with a $5,000 sign-on bonus at Evergreen Crossing, The Loft Healthcare Center, and Southwood Healthcare Center. The PT will evaluate residents, develop and implement treatment plans, and supervise PTAs while collaborating with the rehab team.

The role emphasizes a patient-centered approach with cross-facility opportunities, ongoing education, and a commitment to high-quality post-acute care

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Responsibilities

• Evaluate, review, and screen patient’s/resident’s medical charts and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment and supervision of PTAs.
• Lead interdisciplinary meetings to relay clinical findings and make rehab recommendations.

Job Responsibilities include, but are not limited to:

• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
