Job summary

CommuniCare Rehab in Indianapolis is seeking a Full Time Physical Therapist (PT) to join our Evergreen Crossing and The Loft Healthcare Center teams. You will be an in-house team member delivering resident-focused therapy within a multi-site setting.

The role emphasizes evaluation, treatment plan development, and education of the care plan to the team, with opportunities to work across centers and improve quality of life for residents.

Qualifications

• A PT degree from an accredited program (Bachelor’s, Master’s or Doctorate).
• Valid or pending Indiana State PT license.

Responsibilities

• Evaluate, review, and screen residents’ medical charts and physician-ordered evaluations to develop a tailored treatment plan.
• Implement treatment plans through direct care and educate the plan of care to all team members.
• Provide supervision for PTAs and collaborate with the rehab team during interdisciplinary meetings.
