Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a Full Time Split opening for a Physical Therapist at Evergreen Crossing and The Loft Healthcare Center and Southwood Healthcare Center in Terre Haute, IN.

As an in-house team member, you will evaluate residents, develop treatment plans, supervise PTAs, and participate in interdisciplinary meetings to improve quality of life for seniors.

Qualifications

• Licensure or eligibility to practice physical therapy in Indiana.
• Graduated from accredited PT program with required degree.
• Experience in post-acute or long-term care is a plus.

Responsibilities

• Evaluate, review, and screen residents' medical charts and prepare treatment plans.
• Directly treat residents and supervise PTAs, educating the plan of care to the rehab team.
• Lead interdisciplinary meetings and communicate findings to team and families.
