Job summary

HCA Florida JFK Hospital is seeking an MSW Social Worker to impact patient care across inpatient units and the ED, collaborating with the care team to support patient and family-centered decisions.

The role includes crisis intervention, discharge planning, and resource referrals, with a focus on intensive care units, Trauma, Oncology, NICU, Transplant and ED settings. MSW is required with no travel.

Qualifications

• Master's degree in Social Work (MSW) required.

Responsibilities

• Collaborate with RN case managers, acute care nurses, physicians and insurers to identify and address patient needs.
• Identify barriers to discharge and recovery across financial, emotional, psychosocial, medical and spiritual aspects.
• Maintain knowledge of Medicare, Medicaid and private payer regulations and managed-care concepts.
• Prepare thorough assessments and discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care team, patients, families and caregivers; support patients in decision making.
• Provide psychosocial support and advocacy for end-of-life, crisis, and discharge planning needs.
