HCA Florida JFK North Hospital in West Palm Beach, FL is seeking a Social Worker MSW PRN to join a patient- and family-centered care team. The role focuses on non-clinical biopsychosocial assessments, crisis intervention, discharge planning, and resource referrals across inpatient floors and the ED.

The candidate must hold a Masters Degree in Social Work (MSW) and be prepared to collaborate with care teams, insurers, and families to support safe, timely discharges and quality outcomes.

Formación

• Masters Degree in Social Work (MSW) required.

Responsabilidades

• Non-clinical, biopsychosocial assessments and interventions for inpatient floors and outpatient ED.
• Crisis intervention, patient/family interventions, high-risk screening, and referrals for financial assistance or resources.
• Assist in post-acute care placement and discharge planning for high-risk patients.
• Coordinate meetings between care team and patients/families/caregivers; support patient preferences and values.
