HCA Florida JFK Hospital in Atlantis, FL seeks a Social Worker (MSW) to perform biopsychosocial assessments, support discharge planning on inpatient floors and in the ED, and collaborate with care teams to elevate patient outcomes.

The role emphasizes crisis intervention, family counseling, resource referrals, and post-discharge placement, with a focus on patient-centered care and respectful consideration of each patient’s values.

Qualifications

• Master's Degree required.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurance companies and staff to identify patient needs.
• Identify and address barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid and private payer regulations and processes; understand managed-care concepts and length-of-stay management.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings between care team and patients/families/caregivers; assist in decision making respecting patient preferences.
• Intervene for psychosocial needs and support in coping with chronic or terminal illness; provide counseling, advocacy and referrals.
• Provide emotional support during end-of-life, crisis and complex situations.
