Job summary

Medical City Plano, part of the HCA Healthcare family, is seeking a Social Worker (MSW) for a full-time day shift (8:00am - 4:30pm, Monday to Friday with rotating weekends) to support inpatient and ED populations.

The MSW will conduct non-clinical biopsychosocial assessments, crisis intervention, and discharge planning, coordinating with care teams to meet patient and family goals while navigating regulatory and payer considerations.

Qualifications

• Master's Degree in Social Work required.
• Licensed as a Registered Master of Social Worker in TX.
• Certification in Social Work preferred; Licensed Clinical Social Worker preferred.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurers, and staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery across financial, emotional, psychosocial, medical, and spiritual domains.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts related to discharge planning and fiscal responsibility.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings with care teams, patients, families, and caregivers to support discharge planning.
• Provide psychosocial support and counseling for chronic, acute, or terminal illness, including crisis interventions.
