Qualifications

• Master's Degree in Social Work.
• Current TX MSW license required.
• 1+ year medical social work experience.

Responsibilities

• Collaborate with RN case managers, nurses, physicians and insurers to identify patient needs.
• Perform biopsychosocial assessments and develop discharge plans for high‑risk patients.
• Coordinate Family Care Conferences and support discharge planning goals.
• Provide crisis intervention, emotional support and referrals for financial assistance.
• Maintain knowledge of Medicare/Medicaid and managed-care regulations affecting care.
